ACKNOWLEDGMENT AND ASSUMPTION OF RISK I, the undersigned participant (or parent/legal guardian of the participant if under 18), acknowledge and understand that participating in a martial arts workshop involves inherent risks, including but not limited to physical exertion, contact with other participants, falls, strains, sprains, bruises, fractures, concussions, and other injuries that may result from training activities. I acknowledge that martial arts training may also involve the risk of unforeseen injuries or accidents.
I voluntarily choose to participate (or allow my child to participate) in the martial arts workshop with full knowledge of these risks and assume all responsibility for any injuries, damages, or losses that may occur as a result of participation.
WAIVER AND RELEASE OF LIABILITY In consideration for being allowed to participate in this martial arts workshop, I, on behalf of myself, my heirs, executors, administrators, assigns, or personal representatives, hereby release, waive, and discharge the workshop instructors, organizers, assistants, facility owners, and any affiliated entities from any and all claims, liabilities, damages, or causes of action, whether known or unknown, arising out of or related to my participation in the workshop, including but not limited to injuries, property damage, or losses sustained during training or related activities.
I understand that this release includes claims based on negligence or any other cause of action. I further agree that I will not hold any of the released parties responsible for any costs, including medical expenses, incurred due to injury sustained during participation.
MEDICAL TREATMENT CONSENT I authorize the workshop instructors and organizers to seek emergency medical treatment on my behalf (or my child’s behalf) in the event of an injury or medical emergency. I understand that I am responsible for any medical expenses incurred as a result of participation.
CODE OF CONDUCT AGREEMENT I agree to follow all rules and instructions provided by the instructors to ensure a safe training environment. I understand that failure to comply with safety guidelines or engaging in reckless or aggressive behavior may result in my removal from the workshop without refund.
ACKNOWLEDGMENT AND SIGNATURE I have read and fully understand this waiver and release of liability. I voluntarily agree to its terms and confirm that I am physically fit to participate in martial arts training. If signing on behalf of a minor, I confirm that I am the legal parent/guardian and have the authority to agree to these terms on the minor’s behalf.
