You only need to fill out this form if you are changing your membership level, payment method, and/or payment plan.
 

If you do not fill out this form by August 17, 2026, we will auto-renew your membership at the same level, payment method, and plan.

Membership runs from September 1, 2026 through August 31, 2027.

An annual Security Fee of $300 is included in each membership level according to the payment schedule.

We will place a $1 charge on your card to save your payment information into our system. 

Funds will be charged on the 1st of the month, beginning September 1st, 2026, per the payment plan that you chose.

Chabad of Plano wishes to accept all who want to be members and participate in its programs regardless of financial means.
